Abstract Introduction Total phalloplasty (TP) is used for the creation of a penis in female to male sex reassignment surgery (FTM SRS). Objective We focused our work on free flap transfer technique using latissimus dorsi myocutaneous (MLDF) order create neophallus that allows safe performance urethroplasty and two cylinders prosthesis implantation. Methods Five patient underwent surgery. A necessary precondition this was normal body mass index. The donor site prepared preoperatively by skin massage at least one month before enable easier closure. first surgical step includes removal genitalia (transvaginal hysterectomy, adnexectomy, colpocleisis), proximal urethroplasty, perineoplasty, formation external (scrotoplasty, phalloplasty), 1st stage distal urethroplasty. second tubularizing glans plasty, testicular In third implantation 2-cylinder penile performed through infrapubic approach. time between stages 6-9 months. Results five pts just stages. Penile length varies from 12-18cm girth 11-16cm. One partial loss occurred. urethral stricture developed 3 pts; 1 pt diverticulum.
